For a teacher who probably never heard it
The strange arithmetic of teaching is that the teacher who changed your direction has no idea they did it. You were one of forty that year and one of several thousand across a career, and the moment that mattered to you was, to them, an ordinary Tuesday.
Which is why specificity is the whole gift here. Not "you were a great teacher" — the sentence they have heard until it means nothing — but the actual thing. The comment in the margin. The time they kept you back to explain it again. Name it, and you give them something they genuinely did not have.

Questions people actually ask
Can a whole class send one?
Yes. One person builds it, everyone contributes lines and photographs, and you send the single link to the group. It is easily the most common way these are made.
Is it strange to send this years later?
It is the best version. A message from a student they taught fifteen years ago carries considerably more weight than one from the class currently in front of them.
